This high-touch approach enhances convenience, strengthens patient relationships, and allows individuals to focus on their health while we handle the details.
Essential Plan
Our Essential Plan is a cost-effective option $150 per month (discounted if paid for the year) designed for individuals who are generally healthy and need only routine care. It includes annual wellness visits, same-week appointment access, basic care coordination, and direct messaging for quick questions—all at an affordable rate that prioritizes convenience and preventive care.
Premium Plan
Our Premium Plan is ideal for patients who prefer more frequent interaction and ongoing management of their health. It includes all Essential Plan benefits plus unlimited visits (up to 4 times a month), priority scheduling, extended appointment times, and enhanced access to your provider for personalized, proactive care. $350 per month (discounted if paid for the year). Need to add a spouse or family member +$250 additional.
